Caring and supporting people with a life-limiting illness can place a huge amount of strain on the family unit and give the choice, most people at the end-of-life stage would choose to be at home surrounded by those closest to them.

You want your loved ones to be treated with respect, to maintain their dignity and ensure that their preferences and choices about their daily lives and activities are acted upon.

The service we provide at Solution2Care Services works closely with the family, district nurses and other members of the primary healthcare teams to carefully assess the amount of support and care that is needed and more importantly, acceptable to both the patient and all family members. As a provider of both Registered Nurses and Complex Care Worker, we have the capability to adapt a package of care to suit the changing needs of our service users. Our quality and clinical standards meet best practice requirements set out by the Care Quality Commission to ensure that we effectively manage clinical risks.

Our Complex Care Worker and Registered Nurses are trained to support the emotional, physical and psychological needs of each service user. Our packages of care can lessen the burden on family carers and provide much needed respite care. Our service can be accessed by patients earlier in their illness and not just at the end-of-life stage. We are committed to putting people at the centre of decisions about the care that they receive.

In consultation with you we will provide you with a personalised care plan that will take into account your needs, preferences and choices. We also like to ensure continuity of care by introducing a small number of Complex Care Worker or Registered Nurses to your care package. There often is no defined timeframe in these circumstances but we can be with you to support and care for your loved one however long the journey.
