A serious head injury can leave you with a variety of physical, emotional, behavioural and cognitive needs and these needs differ dependent on the type of injury.

Our care service can support you during your rehabilitation period or provide a package of care to support your on-going needs.

Our care plans are bespoke to the individual need of the service user and we are able to support and care for varying severities of injury. Care focuses on improving patients’ functional abilities so that they can live as independently as possible. We work with the wider multi-disciplinary teams helping to ensure that service users are working towards goals that are meaningful and individual to them.

Solution2Care Services are able to support patients during the transition from in-patient to returning to their home environment. In order to do this successfully we work closely with families, specialists, rehabilitation centres, therapists and other health organisations to tailor our brain injury support to meet the requirements of your needs.

We are able to provide support whether you are going through a staged discharge or returning home on a full time basis.
